发作性睡病等睡眠障碍的遗传机制、免疫调控及睡眠呼吸暂停的远程医疗技术

研究兴趣:
1. 睡眠呼吸暂停的神经调控机制
睡眠呼吸暂停的发生关键在于上气道塌陷,为什么呼吸暂停只在睡眠时发生?明确是解剖结构异常还是功能改变引起直接决定是否需要手术治疗。通过建立人工气道在睡眠状态下测定发现睡眠时中枢调控功能降低是发病的关键,奠定了内科治疗的理论基础。动物实验和治疗反应进一步证实这可能与遗传及长期低氧继发的钝化反应有关。
2.发作性睡病等睡眠疾病的遗传免疫机制
发作性睡病属于罕见病,是脑内食欲素神经元死亡所致。在近30年的研究中,构建了国际上最大的发作性睡病病例和资源数据库;建立了放免法测定脑脊液食欲素水平诊断睡病的方法;发现了国人的遗传易感基因且与免疫相关,证明上呼吸道感染特别是2009年H1N1甲型流感可以诱发睡病,是触发免疫反应的重要抗原;明确了流感病毒诱发睡病的外周及中枢免疫机制。以上结果为免疫治疗奠定了基础。此外,通过国际合作发现了周期性嗜睡(Kleine Levin Syndrome)及不宁腿综合征(Restless Leg Syndrome)的易感基因。
3.睡眠疾病的远程医疗系统构建
通过筛选睡眠呼吸暂停的可穿戴设备,应用人工智能等技术建立了家庭远程医疗系统,验证了该体系的临床效能及医疗经济学价值,实现了转化应用,并在全国各地推广。
